Its flagship program, Alix's Closet, provides free access to gender-affirming clothing, shoes, outerwear, and essential transition garments like chest binders. The closet allows visitors to browse affirming attire in an accepting, pressure-free environment designed to respect individual expression and bodily autonomy. Recognizing the economic instability and food insecurity disproportionately faced by transgender individuals, MTUG also operates an on-site food pantry that distributes shelf-stable meals, fresh produce, and personal care products without income verification or restrictive barriers, ensuring dignified community access to daily essentials.
Transportation equity and emergency support further enhance community stability across the bi-state area. The organization coordinates a bus pass distribution program that provides public transit passes to individuals needing transportation for medical appointments, name-change court dates, employment opportunities, and social gatherings. Community center open hours provide a safe drop-in environment where individuals can access computers, use charging stations, relax in a supportive lounge, and receive referrals to trans-competent physical and mental healthcare providers who offer affirming clinical care throughout Missouri and Southern Illinois.
Peer fellowship and emotional support thrive through a robust network of community-led affinity groups. Facilitated circles meet regularly to support various segments of the gender-expansive community, including groups specifically for transfeminine individuals, transmasculine community members, nonbinary people, and partners of trans individuals.
