Bronx, NY · Serving New York City, Upstate New York, and Puerto Rico

Serves Latino, Black, and low-income individuals, families, seniors, and unhoused residents across New York and Puerto RicoLatino communitiesRegistered nonprofit

Acacia Network is the flagship Latino-founded community development, primary healthcare, and affordable housing institution dedicated to uplifting underserved, immigrant, and low-income families throughout New York City, Upstate New York, and Puerto Rico. Rooted in 1969 through grassroots Puerto Rican community leaders who established Promesa, the Ramon S. Velez health centers, and the Greenpoint mental health clinic to address systemic healthcare disparities and substance addiction in the South Bronx, the agency has grown over more than five decades into one of the largest Hispanic-led nonprofit health and human service organizations in the nation, serving over 150,000 vulnerable individuals each year across an expansive regional footprint of community facilities, health centers, and residential developments.

The network’s cornerstone operational philosophy centers on an innovative Integrated Care Model that simultaneously addresses the social determinants of health. Through its extensive network of Federally Qualified Health Centers (FQHCs) and community health clinics located throughout the Bronx, Manhattan, Queens, and Brooklyn, the agency delivers comprehensive primary care, pediatric medicine, women's reproductive health, dental services, and specialized HIV and hepatitis C management. The organization’s licensed behavioral health division delivers trauma-informed outpatient psychotherapy, psychiatric medication management, and chemical dependency recovery programs, including residential detoxification, medically supervised withdrawal, medication-assisted treatment, and outpatient relapse prevention counseling.

Housing stability and community revitalization form another monumental pillar of the network’s community impact. As one of New York’s largest nonprofit housing developers and operators, the agency manages thousands of units of permanently affordable rental housing, permanent supportive housing with wraparound casework, and transitional shelter facilities for unhoused families, single adults, and veterans. On-site social workers connect tenants with job placement assistance, healthcare navigation, and financial literacy coaching, fostering long-term stability and eviction prevention across diverse neighborhoods throughout the city.

Community enrichment spans all generations through educational and cultural programming. The agency operates accredited early childhood Head Start and daycare centers, afterschool youth STEAM programs, workforce development training for justice-impacted individuals, and vibrant senior centers offering congregate meals and physical wellness. In the cultural arts sector, the network operates the historic Loisaida Center in the Lower East Side, celebrating Puerto Rican, Latino, and diasporic art, music, and social activism through public exhibitions and community festivals that preserve rich neighborhood heritage.

Individuals and families can access comprehensive direct medical care, mental health counseling, housing assistance, and social services by visiting any of the network's neighborhood clinic or housing offices across New York City or calling the central intake center. Bilingual staff members assist clients in Spanish and English without regard to insurance or immigration status. Community partners, healthcare advocates, and philanthropic supporters can advance the mission by partnering on community health drives, joining civic initiatives, or making tax-deductible financial donations to sustain vital community empowerment programs.
